1. Eligibility
The Survey is open to individuals aged 18 or older who are legal residents of Northern Ireland, England, Wales, Scotland, or the Republic of Ireland.Employees of Rephobia, its affiliates, subsidiaries, advertising and promotion agencies, and their immediate family members (spouse, parents, children, siblings, and their respective spouses) and those living in the same household are not eligible to participate.
2. Survey Period
The Survey begins on April 20th, 2024, and concludes on a date to be determined ("TBD") by Rephobia once a minimum of 100 completed Survey responses have been received ("Survey Period").
3. How to Enter
To enter the Survey, complete the online form accessible via www.rephobia.com/surveyOnly one entry per person is permitted. Multiple entries will be disqualified.
4. Prize
One (1) winner will be randomly selected from all eligible entries to receive a £30 One4All gift card ("Prize").The Prize is non-transferable and cannot be redeemed for cash.Rephobia reserves the right to substitute a prize of equal or greater value.
5. Winner Selection and Notification
The winner will be selected within seven (7) business days after the Survey Period concludes.The winner will be notified via the contact information provided in the Survey entry.The winner must claim their Prize within fourteen (14) days of notification. Failure to do so will result in forfeiture of the Prize, and an alternate winner may be selected.
6. Data Collection and Use
What We Collect: By participating, you consent to the collection of personal information, including name, age, contact details, etc. We use your data (A) To run the Survey and select the winner (B) For research on phobias and VR exposure therapy (C) To market Rephobia products and services to you (D) To contact you about Rephobia-related opportunities (E) Sharing: We may share your information with our trusted service providers for the purposes listed above.
7. Intellectual Property
By submitting your Survey responses, you grant Rephobia a non-exclusive, worldwide, royalty-free license to use, reproduce, modify, and distribute your responses for research and marketing purposes.
8. Release of Liability
By participating in the Survey, you release Rephobia, its affiliates, subsidiaries, advertising and promotion agencies, and their respective officers, directors, employees, representatives, and agents from any and all liability for any injuries, losses, or damages of any kind arising from or in connection with the Survey or the Prize.
9. Limitation of Liability
IN NO EVENT SHALL REPHOBIA BE LIABLE FOR ANY INDIRECT, INCIDENTAL, CONSEQUENTIAL, SPECIAL, OR PUNITIVE DAMAGES ARISING OUT OF OR IN CONNECTION WITH THE SURVEY, THE PRIZE, OR THESE TERMS.
10. Disputes
These Terms shall be governed by and construed in accordance with the laws of Northern Ireland.Any disputes arising from or related to the Survey or these Terms shall be resolved exclusively in the courts of Northern Ireland.
11. General Provisions
Rephobia reserves the right to modify or cancel the Survey at any time without notice.If any provision of these Terms is deemed invalid or unenforceable, that provision shall be severed, and the remaining provisions shall remain in full force and effect.By participating in the Rephobia Survey, you acknowledge that you have read, understood, and agree to be bound by these Terms and Conditions.
